You are right its quasi experimental not observational study I went again and read it, the researcher were not observing two groups of treatment, patients who refused larval treatment or didn’t qualify larval treatment were given ozone thats intervention and not just observation.

Edit – study design is qausi experimental, but analysis is an observational cohort. It’s a hybrid study
So generally for Quasi-experimental designs involve intervention without randomization but typically attempt to establish causality using advanced methods like:Interrupted time series, Difference-in-differences, Instrumental variables, Propensity score matching, here they are just measuring associations so that should be kept in mind while reading conclusion.
