If you are in the US then Job Corps or various Americorp programs (NCCC, FEMA Corps, etc) would be good to look into as transitional programs to get you away from home and moving towards independence while still having some structure and assistance along the way and gaining skills you need to get higher paying jobs that can support yourself.

Job Corps can make accommodations for disabilities. If you had a IEP or 504 in high school they can start with that. They would pay to transport you to a center, house you, feed you, pay you a small stipend, and provide basic medical, dental, and mental health services. Mostly they would focus on education and job training. Ideally you would come out with a valuable skilled trade certification or license.

Americorp NCCC/FEMACorp would have you traveling around in a van full of other young people either doing conservation projects or disaster relief. Room and board is covered and you would get a stipend on top. If you serve for a year you would get $7k educational award that can be used for college or vocational training.
