The only time in my entire life I have ever heard of such a thing is when my family was living overseas in Bangladesh, and my daughter learned in school to make spaghetti with ketchup as the sauce. We were horrified.
There are a lot of Americans. Someone, somewhere, has done just about everything horrifying thing you would care (or not) to imagine. But, no, I’ve never heard about that as a commonly done thing.
Ew. I do sub ketchup sometimes for tomatoe paste depending on the recipe. So it’s probably doable but it makes things a little sweeter and I find for homemade spaghetti sauce, you are usually fighting against sweetness so I would not want to try it.
Cheap noodles mixed with ketchup packets taken from McDonald’s seems like something that people who are broke as fuck might do and call “spaghetti”. I don’t think it’s a common culinary decision that people who have other options go for.
The source of the confusion may be because in some places outside the u.s., ketchup is called tomato sauce. In the u.s., tomato sauce is only used to refer to the Italian style marinara sauce traditionally served with pasta.
In the movie The Buttercream Gang, one of the characters’ aunts makes spaghetti that way. I thought it was a joke about how she couldn’t cook. Until I made a new friend whose mom made spaghetti with ketchup. I couldn’t eat it. It was disgusting, but being a people pleaser and taught not to be rude as a guest, I choked down a small serving and declined seconds.

My parents did this for me when I was a very, very young child. Maybe 3 or 4. I was extraordinarily picky and wouldn’t eat pasta sauce because it had chunks in it. Haven’t eaten it since, don’t know anyone over the age of 6 who does. But I have heard of other little kids eating it.
Never! However, I do put ketchup in yakisoba sauce, which is kind of like curly spaghetti, I guess? It’s really good, though, not like just ketchup and spaghetti would be.

I associate ketchup on spaghetti more with countries like Japan that don’t have a strong Italian influence.
