Parent has a credit card in my name (United States)

Long story short my dad made a credit card in my name without my knowledge years ago and I didn’t know about it until a few weeks ago. I did a credit report and found he’s actively using it and paying it off. We do not speak and I want nothing to do with the man. I’ve tried contacting the company the card is with and when I do, they tell me I need the card number in order to talk to a representative. Any other attempts have gotten me the useless ai chat bots or voice recordings. Even though he’s paying it off on time, I don’t want him using my credit to pay for his medical expenses. How do I stop this especially over state lines? I live in Michigan and last I heard, my dad was still living in KY. We both lived in KY when the account was made.
