Why is western health care considered bad ?

I’ve always thought that western countries have the best systems , great infrastructure, smart policies, and a comfortable lifestyle. So why do my friends and family who’ve lived or visited there complain so much about western healthcare? Is it very expensive?
Probably. I’ve heard stories on internet about big medical bills like paying thousands just for an ambulance ride or a broken arm. Even with insurance, people say deductibles and hidden costs drain their wallets. This is confusing to me . How can first-world healthcare be bad?
